An inspectable template
This is an editorial worksheet, not a generated clinical finding or a private physician document. It shows the structure of a useful preparation without making conclusions about a patient or study.

Use the worksheet with a real source
Start with a paper saved for a recurring department question. An appraisal field may read 'Allocation details: not established from the available abstract.' That is an access limit, not a positive or negative bias judgment. Add a judgment only when you can identify supporting source material.
Paper identity and study design
Record the source identifier and the design the paper actually describes. What was the prespecified research question? Identify any related reports.
Abstract or full-text access
List the material you read, including any supplement or study protocol. Which appraisal details cannot be established from the available source?
Population and applicability
Describe eligibility, setting, baseline characteristics and follow-up. State your research setting separately. Which source-supported differences matter?
Methods and risk of bias
What supports the allocation, measurement, missing-data and analysis judgments? Record the relevant passage for each judgment. Leave unavailable methods unresolved.
Results with source support
Record outcome, time point, denominator, estimate and reported uncertainty. Attach the result's table or passage. Identify deviations from the planned analysis.
Limits and your interpretation
Separate what the paper reports from your assessment of its relevance. What remains uncertain, and which source or question would resolve it?
Review the completed work
Place the worksheet beside the paper and any AttendMe appraisal. Use it to check the basis of each important point. Keep reported findings, supported methods judgments and your applicability interpretation distinct.
Use it with an original source
Fill the worksheet from material you can access, record source limitations and keep your interpretation distinct from reported results. The downloadable template uses the same sections shown here.
